Students can qualify for an immunization exemption for required vaccines based on religious or medical grounds. To submit an exemption for the fall semester forms must be completed by August 1.
Medical exemption
To apply for a medical exemption for a required vaccine, students must fill out the Medical Immunization Exemption form (PDF) and submit it to Health Services. Please note that your medical provider (MD/DO, NP or PA) must fill out the 2nd and/or 3rd page of the form and sign it.
Religious exemption
To apply for a religious exemption for a required vaccine, students must fill out the Religious Immunization Exemption form. Submissions will be reviewed on a rolling basis, and you will receive a response via the Student Health Portal within 10 days of your submission.
